Direct Payment Plan

Patients who are either cash pay or who are enrolled in commercial insurance plans will have the option to enroll in our direct pay plan. This will allow patients greater access to primary care services, without the unexpected burden of an out-of-pocket expense in the form of copayment or balance billing.

Benefits of Direct Payment Plan:

• Monthly premium which can be cancelled at any time
• Replaces fee-for-service billing
• Contract fee covers the cost of many primary care services
• Removes additional financial barriers the patient may encounter in accessing routine primary care, including prevention, wellness, and chronic care services
• Many patients still carry insurance coverage for high burden expenses such as imaging and hospital admission

Cost Structure:

• $50 per month ages 21-44
• $75 per month ages 45-65
• $100 per month ages 66 and above
• $10 per child per month (parent must be a member)

Includes:

• Annual medical exam (with basic labs)
• Treatment of chronic and acute conditions
• Cancer screenings:
  • Cervical (PAPs)
  • Prostrate (PSA)
  • Colon (Stool FIT test)
• Referral for mammogram and colonoscopy
• Steroid injections
• Breathing treatments
• Wound care
• Telemedicine visits
